Medicine is a vast and complex field that is difficult to summarize briefly. Preventing illness and optimizing treatment are among the most critical factors for long-term health. It is common for one health issue to lead to another; illnesses often cluster together due to shared risk factors, biological mechanisms, or side effects from medications.

While there has been significant progress in many areas, I still frequently observe inefficiencies or a lack of attention to essential factors in clinical practice. Key areas that require a more comprehensive approach include:

Mental health and well-being. There is a need for more effective use of therapeutic methods, neuromodulation, and a truly holistic approach to psychiatric care.

Management of primary risk factors. Clinicians must address fundamental risks more aggressively by conducting thorough investigations and ensuring patients are fully informed about their individual risk and metrics, specifically blood pressure, cholesterol, blood sugar, and liver function.

Hormonal and respiratory optimization. Care should be refined regarding asthma and thyroid medications, contraceptive methods, menopause management, and the optimization of iron levels.

Digestive health. A comprehensive approach to gastric symptoms is vital, involving sufficient testing and a systematic review of all available treatment options.

Obesity management. Effective treatment of obesity requires a combination of nutritional guidance, pharmacological support, and surgical intervention when appropriate.

Sleep disorders. Obstructive sleep apnea remains underdiagnosed. Sleep medicine should utilize a personalized range of methods, from medication to sleep hygiene and therapeutic techniques.

Bone health. Proactive maintenance of bone density is essential through the use of appropriate medications and nutritional supplements.

Chronic pain. The underlying causes of long-term pain should be investigated using tools like ENMG or MRI. Treatment must be multifaceted, incorporating physiotherapy, neuromodulation, psychotherapy, injections, mindfulness, education, and medication.

Skin lesions. This includes both cancerous changes and those that cause functional impairment.

Challenges with polypharmacy. Managing multiple medications is a well-recognized challenge. It is often beneficial to periodically re-evaluate the benefits and side effects of ongoing drug treatments.

Sexual health. Although this can be a difficult topic to discuss, it remains highly significant for overall quality of life. There is a wide range of effective treatment options available for various concerns.
